REDMAP conversation cards
PATCH funded 3,000 pocket prompt cards (and a downloadable version) so hospital staff can use the REDMAP conversation framework when planning future care with patients and families.
Finding the right words
PATCH funded the production of 3,000 bankcard-sized folded cards, plus a downloadable version, so hospital staff could keep a quick reference to the REDMAP conversation framework (Ready, Expect, Diagnosis, Matters, Actions, Plan) to hand on busy wards.
The cards were a practical launch aid: pocket prompts to help clinicians open care-planning conversations, talk about changes in health, discuss what matters to the person, and navigate decisions including cardiopulmonary resuscitation (CPR).
The support from PATCH helped us launch the cards at a time when staff needed advice and support in having meaningful discussions.
Where to get the cards
Current REDMAP and future care planning materials are available from the SPICT programme and NHS Education for Scotland (TURAS). The pocket cards from the PATCH-funded print run are still used in many places; newer Future Care Planning cards are published on TURAS.
